Simplifying Advance Care Planning

Advance care planning (ACP) is often seen as a dry and complex topic, yet it is essential for both health professionals and consumers to engage with in order to increase its uptake.
 
The process involves extensive documentation and reading, which can be overwhelming. However, the WA Department of Health has made significant efforts to simplify it by providing clear guidelines and creating videos that streamline the process for both health professionals and the public. In addition to funding Palliative Care WA (PCWA) to deliver engaging consumer workshops.

Recognising the need for an innovative approach to educate health professionals on the importance and impact of advance care planning, the Clinical Lead of the End-of-Life Care Program sought a fresh perspective, which PaSCE delivered upon.

Power of Visual Learning

Hearth Care has emerged as a powerful and widely appreciated resource, known for its creative and engaging sketch notes that simplify complex palliative care topics.

These visually appealing tools have been successfully used in specialist palliative care education and have received positive feedback from health professionals.

Building on this success, we saw an opportunity to further enhance advance care planning education across WA.

A Collaborative Approach

With this in mind, we reached out to Hearth Care to explore the development of an Advance Care Planning (ACP) Sketch Note Series.

This collaboration aims to make ACP more accessible, engaging, and impactful for health professionals, ensuring they feel more confident in discussing and implementing these crucial conversations.

Health professionals have an important role in supporting people through advance care planning and following the documents when a person loses capacity.

Anyone over 18 is recommended to create their advance care plan.


This poster is a novel and broad explanation of “what, who, how, and when” advance care planning is relevant.

Advance care planning is a voluntary process of planning for future health and personal care whereby the person’s values, beliefs and preferences are made known to guide decision-making at a future time when that person cannot make or communicate their decisions.
