EDUCATION overview
This online education provides a foundational understanding of Advance Care Planning (ACP) documents and their role in supporting care. Participants will understand that whilst the Goals of Care (GoC) form does not replace an Advance Health Directive (AHD) it can be used in conjunction with it.

By completing this online education, participants will develop knowledge and confidence to effectively embed ACP into their practice, supporting the delivery of person-centred care.
